feat: implement ntfy-discord bridge in Go
- SSE subscription to ntfy with auto-reconnect
- Discord webhook integration with embed formatting
- Priority to color mapping, tag to emoji conversion
- Native HashiCorp Vault support (Kubernetes + token auth)
- Hot reload secrets via fsnotify or Vault polling
